Abstract

Kutipan Akta Kelahiran merupakan dokumen administrasi kependudukan yang dibuat dan diterbitkan oleh instansi yang berwenang dalam rangka penataan data kependudukan melalui pendaftaran penduduk sebagaimana telah diatur dalam Undang-Undang Administrasi Kependudukan No. 24 Tahun 2013 dalam penerbitan dokumen administrasi kependudukan tersebut pejabat yang berwenang untuk membuat dan menerbitkan dokumen tersebut tidak luput dari adanya kesalahan pengetikan, sehingga dalam kurun waktu 3 bulan permohonan mengenai perbaikan kesalahan pada akta kelahiran di PN Batam Kelas 1A mencapai 382 perkara. Dalam proses pemeriksaan di persidangan permohonan yang dibuat oleh pemohon sering tidak bersesuaian dengan apa yang sebenarnya dibutuhkan oleh pemohon tersebut, sehingga hakim pemeriksa sering menyarankan pemohon untuk merubah atau mencabut permohonannya, dan hal tersebut tentu sangat merugikan pemohon dalam bentuk materiil maupun immateriil. Laporan kerja praktek penulis lakukan berdasarkan penelitian lapangan yang mengacu pada data primer dan data sekunder. Tahapan dalam perancangan laporan kerja praktek terdiri dari tahap awalan (melakukan riset), penerapan (menganalisa data dan merancang luaran proyek), penilaian (menilai kesesuaian luaran proyek yang telah di rancang) dan pelaporan. Format Permohonan Perbaikan Kesalahan Pada Akta Kelahiran yang baik dan benar sesuai dengan kebutuhan pemohon ialah luaran proyek yang penulis hasilkan dengan tujuan memudahkan masyarakat dalam membuat permohonan perbaikan kesalahan pada akta kelahiran dan memberikan informasi mengenai apa yang harus dilengkapi pada saat mendaftarkan permohonan. Hasil dari implementasi luaran proyek ini membantu masyarakat dalam membuat permohonannya yang akan diajukan ke PN Batam Kelas 1A. ********************************************************************** Birth Certificate Quotation is a population administration document made and issued by the competent agency in the framework of structuring population data through population registration as stipulated in Population Administration Law No. 24 of 2013 in the issuance of population administration documents, the authorized officer to produce and publish these documents did not escape the typing errors, so that within a period of 3 months the application for correcting errors on birth certificates in Batam PN Class 1A reached 382 cases. In the examination process at the hearing the requests made by the applicant often do not correspond to what is actually needed by the applicant, so the examining judge often advises the applicant to change or revoke his application, and this is certainly very detrimental to the applicant in material and immaterial form. The author's practical work report is based on field research that refers to primary and secondary data. The stages in the design of practical work reports consist of the prefix (conducting research), application (analyzing data and designing project outcomes), assessment (assessing the suitability of project outputs that have been designed) and reporting. Format of Requests for Correcting Errors on Birth Certificates that are good and right according to the needs of the applicant is the outcome of the project that the author produced with the aim of making it easier for the public to make requests for correction of errors on the birth certificate and provide information about what must be completed when registering the request. The results of the implementation of this project output helped the community in making their application which would be submitted to the Batam PN Class 1A.
